It is contradictory that there are non-flat individuals in a flat mechanism that determines policies through discussions. But in reality, that’s what you can’t rule out. With that in mind, a flat organization doesn’t really exist. An organization is just a group of individuals to take responsibility.

There is a word we often hear
when someone does something, fails, and causes damage to the organization they belong to. That was, “I did it because I was instructed.” In other words, it’s the theory that the person who gave the instructions is responsible for the damage.
Certainly, those who must receive and execute instructions do something only because they obey the instructions. But should responsibility for the loss be taken only by the person who issued the instructions? Does the person who did it directly need not take responsibility?
In other words, even if you follow the instructions, shouldn’t the implementor take some of the responsibilities if you could reasonably assume that doing so would result in a loss? I did it because I was instructed. But who embodies the words of instruction?
Organizations make decisions through discussions among multiple people. However, the organization itself does not take responsibility when problems arise. It is someone in the organization who takes responsibility. In other words, an organization is a system that moves beyond the individual but leaves the responsibility to the individual.
